Association between sex and perioperative opioid prescribing for total joint arthroplasty: a retrospective population-based study

چکیده

BackgroundScarce data exist on differential opioid prescribing between men and women in the pre-, peri-, postoperative phases of care among patients undergoing total hip/knee arthroplasty (THA/TKA).MethodsIn this retrospective population-based study, Truven Health MarketScan claims were used to establish differences (1) year before THA/TKA surgery, (2) amount opioids prescribed at discharge, (3) chronic (3–12 months after surgery). Multivariable regression models measured odds ratios (OR) with 95% confidence intervals (95% CI).ResultsAmong 29 038 THAs (42% men) 48 523 TKAs (52% (compared women) less likely receive an prescription surgery (54% vs 60%, 54% 60% for THA TKA, respectively); P<0.001. However, multivariable analyses male sex was associated higher dosages discharge (OR=1.04; CI 1.03, 1.06) TKA (OR=1.05; 1.04, 1.06); both Chronic found 10% cohort (THA: n=2333; TKA: n=5365). Here, demonstrated lower persistent specifically (OR=0.90; 0.82, 0.99) but not (OR=0.96; 0.90, 1.02); P=0.026 P=0.207, respectively.ConclusionsWe sex-based across all THA/TKA. The results highlight temporal opportunities targeted interventions improve outcomes joint arthroplasty, particularly women, decrease prescribing.

Perioperative Pain Management Following Total Joint Arthroplasty

Total Joint Arthroplasty (TJA) has proven to be very successful in the treatment of patients suffering from endstage osteoarthritis of the hip and knee [1,2]. The demand for primary TJAs is expected to dramatically increase over the next several years due to more procedures being performed annually, younger patients being considered candidates for surgery, and patients exhibiting longer life ex...

Sleep apnea and total joint arthroplasty under various types of anesthesia: a population-based study of perioperative outcomes.

BACKGROUND AND OBJECTIVES The presence of sleep apnea (SA) among surgical patients has been associated with significantly increased risk of perioperative complications. Although regional anesthesia has been suggested as a means to reduce complication rates among SA patients undergoing surgery, no data are available to support this association. We studied the association of the type of anesthesi...

Long-Acting Opioid Use Independently Predicts Perioperative Complication in Total Joint Arthroplasty.

BACKGROUND Opioid therapy is an increasingly used modality for treatment of musculoskeletal pain despite multiple associated risks. The purpose of this study was to evaluate how preoperative opioid use affects early outcomes after total joint arthroplasty. METHODS A total of 174 patients undergoing total joint arthroplasty were matched by age, gender, and procedure into 3 groups stratified by...

Association between opioid prescribing patterns and opioid overdose-related deaths.

CONTEXT The rate of prescription opioid-related overdose death increased substantially in the United States over the past decade. Patterns of opioid prescribing may be related to risk of overdose mortality. OBJECTIVE To examine the association of maximum prescribed daily opioid dose and dosing schedule ("as needed," regularly scheduled, or both) with risk of opioid overdose death among patien...

Perioperative stroke after total joint arthroplasty: prevalence, predictors, and outcome.

BACKGROUND The risk of perioperative stroke following cardiac and carotid artery surgery is well documented. There is an apparent lack of recognition and appreciation of this complication after total joint arthroplasty. The present study was designed to determine the prevalence of, and outcome after, perioperative stroke following total joint arthroplasty. In addition, risk factors for the deve...
