Assessment of endogenous fibrinolysis in clinical practice using novel tests: ready for clinical roll-out?

چکیده
Abstract The occurrence of thrombotic complications, which can result in excess mortality and morbidity, represent an imbalance between the pro-thrombotic fibrinolytic equilibrium. mainstay treatment these complications involves use antithrombotic agents but despite advances pharmacotherapy, there remains a significant proportion patients who continue to remain at risk. Endogenous fibrinolysis is physiological counter-measure against lasting thrombosis may be measured using several techniques identify higher risk benefit from more aggressive pharmacotherapy. However, assessment system not yet accepted into routine clinical practice. In this review, we will revisit different methods assessing endogenous (factorial assays, turbidimetric lysis viscoelastic global tests), including strengths, limitations, correlation outcomes each method how might integrate practice future.

Is IORT ready for roll-out?
Two large randomised controlled trials of intraoperative radiotherapy (IORT) in breast-conserving surgery (TARGIT-A and ELIOT) have been published 14 years after their launch. Neither the TARGIT-A trial nor the ELIOT trial results have changed the current clinical practice for the use of IORT. The in-breast local recurrence rate (LRR) after IORT met the pre-specified non-inferiority margins in ...
متن کاملUsing HIV resistance tests in clinical practice.
Genotypic resistance testing is now a standard of care in HIV management. Although there are clear, published guidelines to recommend the appropriate use of these tests, clinicians and scientists still struggle to determine the optimal use of resistance tests given the finite budgets and time constraints under which they work. In this article we discuss some 'real-life' clinical situations and ...

Safe and eff ective vaccines to prevent Ebola infection would be useful in the fi ght against this devastating disease. Depending on their effi cacy, onset of immunity, length of protection, and cost and ease of administration, these vaccines could contain or even prevent an outbreak. During an outbreak, a vaccine could prevent infection of front-line health workers and staff who are engaged in...
متن کاملAre Genetic Tests for Atherosclerosis Ready for Routine Clinical Use?
In this review, we lay out 3 areas currently being evaluated for incorporation of genetic information into clinical practice related to atherosclerosis. The first, familial hypercholesterolemia, is the clearest case for utility of genetic testing in diagnosis and potentially guiding treatment. Already in use for confirmatory testing of familial hypercholesterolemia and for cascade screening of ...
