Assessing neurological prognosis in post-cardiac arrest patients from short vs plain text EEG reports: A survey among intensive care clinicians

چکیده

BackgroundElectroencephalography (EEG) patterns are predictive of neurological prognosis in comatose survivors from cardiac arrest but intensive care clinicians dependent neurophysiologist reports to identify specific patterns. We hypothesized that the proportion correct assessment would be higher short statements confirming EEG compared with descriptive plain text reports.MethodsVolunteering at two university hospitals were asked assess a fictional patient high neuron enolase. They presented 17 authentic and three statements, whether “highly malignant”, “malignant” or “benign” pattern was present. Primary outcome who correctly identified poor consistent highly malignant Secondary outcomes how assessed benign patterns.ResultsOut 57 participants, by 61% 93% statement malignant” Unaffected 28% 40% Good 64% pattern.ConclusionStandardized statement, present”, as descriptions reports, is associated more accurate identification arrest.

Improving neurological outcomes post-cardiac arrest in a rat model: immediate hypothermia and quantitative EEG monitoring.

OBJECTIVES Therapeutic hypothermia (TH) after cardiac arrest (CA) improves outcomes in a fraction of patients. To enhance the administration of TH, we studied brain electrophysiological monitoring in determining the benefit of early initiation of TH compared to conventional administration in a rat model. METHODS Using an asphyxial CA model, we compared the benefit of immediate hypothermia (IH...

Caring for patients receiving therapeutic hypothermia post cardiac arrest in the intensive care unit.

Survivors of ventricular fibrillation cardiac arrest have poor and often devastating neurological outcomes despite advances in resuscitation techniques and services (Bernard et al., 2002; Collins & Samworth, 2008). In an effort to increase survival rates, improve neurological outcomes and reduce mortality for surviving patients, clinical trials have shown that a mild state of therapeutic hypoth...

The Aassociation between Pre-Cardiac Arrest Comorbidity and Unsuccessful Cardiopulmonary Resuscitation in Patients with Cardiac Arrest

Background and Objective: Patients suffering from cardiac arrest (CA) have poor prognosis and survival. The association of pre-arrest comorbidity with unsuccessful resuscitation in patients with CA is far from clear. The aim of the present study was to investigate the association between pre-existing comorbidity and unsuccessful resuscitation following CA in Iranian patients. Materials and Met...

Continuous evaluation of neurological prognosis after cardiac arrest.

Post-resuscitation care has changed in the last decade, and outcome after cardiac arrest has improved, thanks to several combined measures. Induced hypothermia has shown a treatment benefit in two randomized trials, but some doubts remain. General care has improved, including the use of emergency coronary intervention. Assessment of neurological function and prognosis in comatose cardiac arrest...

EEG and cardiac arrest: Divining prognosis at the bedside.

Out-of-hospital cardiac arrest affects 326,200 persons in the United States each year. Mortality remains high despite advances in critical care and emergency response, with only 31.4% of patients surviving to hospital discharge. Survivors also face the prospect of disability due to hypoxic-ischemic brain injury. Prognostication is an important aspect of care in these patients, so that decisions...
