Anesthesia Method, Tourniquet Use, and Persistent Postsurgical Pain after Total Knee Arthroplasty: A Prespecified Secondary Analysis of a Randomized Trial
نویسندگان
چکیده
Background Persistent postsurgical pain after total knee arthroplasty is a common problem and major reason for patient dissatisfaction. This secondary analysis aimed to investigate the effects of anesthesia (spinal vs. general) tourniquet use on persistent arthroplasty. Methods In this previously presented parallel, single-center, randomized trial, 404 patients scheduled were spinal versus general no-tourniquet groups. Patients assessed using Brief Pain Inventory–short form preoperatively 3 12 months postoperatively. The prespecified main outcome was change in “average pain” measured with numerical 0 10 rating scale 1 yr threshold clinical importance between groups set 1.0. Results average scores postoperatively did not differ (–2.6 [SD 2.5] –2.3 2.5], respectively; mean difference, –0.4; 95% CI, –0.9 0.1; P = 0.150). group reported smaller decrease than (–2.1 2.7] –2.8 2.3]; 0.6; 0.1 1.1; 0.012). After yr, concerning four severity variables (numerical scale) decreased more (–2.3 2.2] –1.8 2.1]; –0.5; –0.05; 0.029) less (–1.7 2.3] 2.0]; 0.2 1.0; 0.005). None differences reached importance. Distinguishing problematic from nonproblematic postsurgical pain: a pain trajectory analysis after total knee arthroplasty.
The goal of this study was to follow a cohort of patients undergoing total knee arthroplasty over time to: (1) identify and describe the various pain trajectories beginning preoperatively and for up to 12 months after surgery, (2) identify baseline predictors of trajectory group membership, and (3) identify trajectory groups associated with poor psychosocial outcomes 12 months after surgery. On...
متن کاملPain control after total knee arthroplasty: a randomized trial comparing local infiltration anesthesia and continuous femoral block
BACKGROUND AND PURPOSE Pain after total knee arthroplasty (TKA) is usually severe, and epidural analgesia or femoral nerve block has been considered to be an effective pain treatment. Recently, local infiltration analgesia (LIA) has become increasingly popular but the outcome of this method regarding the analgesic effect has not been fully evaluated. We compared local infiltration analgesia and...
متن کاملPredictors of persistent pain after total knee arthroplasty.
BACKGROUND Several studies have identified clinical, psychosocial, patient characteristic, and perioperative variables that are associated with persistent postsurgical pain; however, the relative effect of these variables has yet to be quantified. The aim of the study was to provide a systematic review and meta-analysis of predictor variables associated with persistent pain after total knee art...
متن کاملCauses of Aseptic Persistent Pain after Total Knee Arthroplasty
BACKGROUND Persistent pain after total knee arthroplasty (TKA) is dissatisfying to the patient and frustrating to the surgeon. The purpose of this study is to evaluate the aseptic causes and clinical course of intractable pain following TKA. METHODS Of the total 2,534 cases of primary TKA reviewed, 178 cases were classified as having aseptic persistent pain that was not resolved within 1 year...
متن کاملPregabalin and pain after total knee arthroplasty: a double-blind, randomized, placebo-controlled, multidose trial.
BACKGROUND Pregabalin may reduce postoperative pain and opioid use. Higher doses may be more effective, but may cause sedation and confusion. This prospective, randomized, blinded, placebo-controlled study tested the hypothesis that pregabalin reduces pain at 2 weeks after total knee arthroplasty, but increases drowsiness and confusion. METHODS Patients (30 per group) received capsules contai...
