Analysis of Space Launch Vehicle Failures and Post-Mission Disposal Statistics

Abstract In a time in which significant number of launch vehicles are being developed, boosted by numerous new space actors demanding cheaper access to space, the need ensure reliability and performance successful Post-Mission Disposal (PMD) is undeniable. The first step analyse previous failures, learning from past mistakes. this paper, failures that occurred over 16 years analysed. These classified as function subsystems involved failure, well phase mission failure occurred. As result, most critical identified for every phase. Subsequently, subsystem further investigated, its components identified. Furthermore, aiming improve present PMD practices, an understanding current status necessary. This paper provides overview evolution terms mean orbital lifetime rocket bodies launched since year 2000. Moreover, manoeuvres performed saved these analysed between 2016 2020, classifying statistics launcher family country.
