An unusual radiological manifestation of a mandibular central giant cell granuloma

Central giant cell granuloma (CGCG) is an uncommon benign osseous lesion of the jaw. We present case aggressive CGCG in a 16-year-old boy with 5-month history facial swelling and nasal obstruction. He was referred for radiology consult multiple expanding swellings on right hemi-face. The were reported to have insidious onset, slow progression no association paresthesia, discharge, or systemic symptoms. Extra oral examination revealed diffuse side face which obliterated nasolabial fold causing asymmetry. A neck computed tomography (CT) showed expansile multi loculated lytic lesions mandibular ramus involved condyle hard palate. Similar also found skull base. Scans fine internal septations ground glass haze. It involvement several teeth roots surrounding soft tissues. pre-operative intra-oral biopsy multinucleated cells, comprising both spindle-shaped round cells. Differential diagnosis helped determine as CGCG. This helps demonstrate wide variation clinical, radiological, histopathological features importance thorough investigation timely diagnosis.
