An unusual case of Miller Fisher syndrome with unilateral 3<sum>rd</sup> nerve palsy and rapid recovery following intravenous immunoglobulin

Miller Fisher syndrome (MFS) is an immune mediated neuropathy commonly triggered by infection. It considered as a variant of Guillain-Barre syndrome. Although bilateral ophthalmoplegia the commonest ocular involvement in MFS, there are reported cases unilateral ophthalmoplegia. MFS has benign course and treatment not required majority unless life threatening complication. We report unusual case presented with isolated right sided oculomotor nerve palsy pupillary involvement, ataxia areflexia. Patient was treated intravenous immunoglobulin (IVIG) achieving remarkable recovery five days treatment. believe this effect rather than natural her illness. it rare, should be included differential diagnosis 3rd especially when associated or Having low threshold to initiate IVIG may accelerate positive impact on patient’s functional status.
