Aggressive localized juvenile spongiotic gingival hyperplasia (LJSGS) in a 33-year-old patient: a case report

The aim of this study is to report a case an extensive and aggressive gingival lesion, compatible with localized juvenile spongiotic hyperplasia (LJSGH), demonstrating the difficulties diagnosis management. A 33 years-old male patient was referred Department Oral Maxillofacial Surgery at Erasto Gaertner Hospital lesion on anterior maxillary gingiva, 3-months evolution period. had complete permanent dentition good oral hygiene. involved gums teeth 11 12, solitary, asymptomatic, red, flaccid bleeding palpation. No palpable lymph nodes were found. An incisional biopsy performed, anatomopathological result "squamous mucosa pseudoepitheliomatous lymphoplasmocytic infiltrate neutrophils". fully excised confirmed showing no malignancy in sample negative for investigation fungus. Biofilm control, mouth rinses hydrogen peroxide, Chlorhexidine Digluconate 0.12% Amoxicilin 500mg+Potassium Clavulanate 125mg have not demonstrated efficacy, kept recurrence short time even after excision. Against situation, infectologist, by suspicion resistant microorganism. He started antibacterial therapy Sulfamethoxazole 800mg+Trimethoprim 160mg that showed results 3 months. Currently, remains follow-up without signs recurrence. LJSGH clinicopathological benign entity appears young patients as soft, reddish mass, mainly buccal gingiva upper maxilla lonely most cases. management condition very difficult because few cases described literature, establishing correct essential ensure appropriate treatment followed.
