The article presents a current view of the etiology and pathogenesis of, adhesive capsulitis shoulder joint basic principles conservative surgical treatmen. Idiopathic is self-limiting disease with gradual improvement in symptoms, sometimes demanding treatment. Currently, role both inflammatory fibrotizing processes generally recognized, when process ultimately leads to fibrotic changes. associated diabetes mellitus, thyroid disease, cerebrovascular coronary heart autoimmune diseases, Dupuytren’s contracture. In theliterature there isno consensus on unified treatment modality for capsulitis: conservative, operative, or combined. number patients, achieved spontaneously, recommended methods range from follow-up invasive open capsulotomy. There no universal algorithm, so should be individualized. By all accounts, first choice includes physical therapy combination physiotherapy, anti-inflammatory drugs, corticosteroid injection, hydrodilation. Surgical indicated patients persistent symptoms ineffectiveness manipulation under anesthesia / capsulotomy (arthroscopic open). Treatment remains an unresolved clinical problem. existing regimens are not further studies long-term outcomes needed develop more effective modality.
