ObjectiveAccessory cavitated uterine mass (ACUM) is characterized by an endometrium-lined cavity surrounded myometrium, usually at the level of round ligament lateral to otherwise normally-shaped uterus [1]. Etiology likely congenital, possibly related gubernaculum dysfunction [2].CaseTwo cases ACUM are presented, describing a rare but important entity in workup dysmenorrhea and pelvic pain. Our first patient was 13 years. with mild who developed episodes 10/10 right-sided pain three years after menarche. MRI showed 2cm collection containing blood products right aspect uterus, myometrium. The second 16 years, severe associated nausea. demonstrated 2.6 × 2.3cm lesion body hemorrhagic material.
