Absolute coronary flow and microvascular resistances in severe aortic stenosis: invasively quantification

Abstract Background Chest pain in aortic stenosis (AS) has been attributed to the loss of coronary flow reserve however; this not directly quantified date. Purpose To determine whether there is a decrease as cause chest or dyspnea patients with severe AS Methods Patients and normal left ventricular ejection fraction (LVEF) underwent coronariography previous valve replacement. In those without obstructive disease, an invasive physiological evaluation was performed measure absolute (Q) microvascular resistances (R). Left anterior descending artery (LAD) explored all cases and, subgroup patients, we also studied circumflex (CX). Results 51 were included. 86% presenting dyspnea, 26% reported pain, 11.8% syncope. No patient admitted due heart failure. Mean pressure gradient for 52.44±15.41 mmHg mean area 0.76±0.17 cm2. LVEF 63±8.35%. The study LAD successfully completed patients. 68.6% presented / increased LAD-Q 262.118±161.99 mL/min. LAD-R proved 56.9% low 13.7%. 400.78±195.89 UW.
