چکیده
Circumferential, continuous, transmural atrial lesions are desired to facilitate pulmonary vein (PV) isolation. However, wall thickness around the PV is variable (5-6 mm in superior/anterior segments and 2-4 inferior/posterior segments). Previous experimental studies have shown that radiofrequency (RF) lesion depth diameter can be predicted by a logarithmic formula using contact force (CF), RF power application time (Force-Power-Time Formula, Ablation Index, AI). The purpose of this study was test ability AI guided ablation achieve left (LA) box isolation single encirclement examine location residual conduction (gap) within patients with fibrillation (AF). In 42 AF, LA performed 7.5Fr CF catheter (ThermoCool SmartTouch SF, Biosense Webster) CARTO system. Using (35-40Watts) (5-45g), target value 380-400 used for posterior/inferior an inter-lesion distance (ILD) 4mm 550-600 anterior/superior/septal ILD 6mm along circumference (Figure). delivered until reached values or terminated prematurely when esophageal temperature <38.5°C. After first line, activation mapping confirm identify gap locations. Total 27.2±4.6 min length 21.2±2.5 cm. Complete obtained 17/42 (40%) pts. remaining 25/42 (60%) pts, set identified earliest line only 4/42 (10%) pts (indicating endocardial gaps), whereas at site distant from 21/42 (50%) suggesting epicardial through septopulmonary bundle Focal gaps focal breakouts resulted compete all 25 effectively prevented line. via often observed (50%).

Prevalence of fever in patients undergoing left atrial ablation of atrial fibrillation guided by barium esophagraphy.
BACKGROUND Real-time esophageal imaging is critical in avoiding esophageal injury. However, the safety of esophageal imaging with barium has not been specifically explored. METHODS Three hundred seventy consecutive patients underwent left atrial (LA) ablation of atrial fibrillation (AF) under conscious sedation. One hundred eighty-five patients (50%) underwent the ablation procedure with, and...
متن کاملAssociation Between Left Atrial Stiffness Index and Atrial Fibrillation Recurrence in Patients Undergoing Left Atrial Ablation.
BACKGROUND Atrial fibrillation (AF) is associated with significant abnormalities of left atrial (LA) systolic and diastolic function. This study describes a novel measure, LA stiffness index, that estimates LA diastolic function and its association with clinical outcomes of catheter ablation. METHODS AND RESULTS A total of 219 AF patients referred for ablation (59% paroxysmal, mean CHA2DS2VAS...
متن کاملAtrial Fibrillation Ablation by the Epicardial Approach.
Catheter ablation is an effective treatment for atrial fibrillation (AF). However, over long-term follow-up, it is becoming clear that AF often recurs and repeat ablation is required. With the goal of improving efficacy, particularly in those patients with risk factors for poor outcomes using a standard endocardial ablation technique, surgical epicardial ablation has emerged as an alternative m...
متن کاملLeft atrial epicardial adiposity and atrial fibrillation.
BACKGROUND Atrial fibrillation (AF) has been linked to inflammatory factors and obesity. Epicardial fat is a source of several inflammatory mediators related to the development of coronary artery disease. We hypothesized that periatrial fat may have a similar role in the development of AF. METHODS AND RESULTS Left atrium (LA) epicardial fat pad thickness was measured in consecutive cardiac CT...
متن کاملradiofrequency atrial fibrillation ablation technique in patients with mitral valve surgery and left atrial reduction procedures
background: about half of all patients who undergo mitral valve surgery suffer from atrial fibrillation (af). cox described the surgical cut-and-sew maze procedure, which is an effective surgical method but has some complications. this study was designed to evaluate the efficacy of a substitution method of radiofrequency ablation (rfa) for patients undergoing mitral valve surgery with af. metho...
