A sporadic case of holocord tuberculous transverse myelitis with arachnoiditis

Abstract Background Tubercular infection of the brain and spine is relatively common in endemic regions world. Central nervous system tuberculosis can have varied manifestations. The familiar imaging findings are hydrocephalus, ring-enhancing tuberculomas, meningeal enhancement, having a preference for basal regions. Myelitis most manifestation spine, with holocord involvement being rare presentation, as seen our case. Case presentation We present case pediatric patient undergoing treatment tubercular brain. developed acute onset quadriparesis, manifesting transverse myelitis on imaging. manifested enhancement non-communicating managed by shunt placement. As latest, follow-up has stable disease course. Clinical laboratory investigations excluded other infectious non-infectious causes myelitis, including neuromyelitis optica spectrum disorders. Conclusions Longitudinally extensive complication long-segment signal abnormality swelling cord corresponding post-contrast enhancement. Involvement entire rare, handful cases reported literature.
