Introduction: Strongyloidiasis is a systemic disease caused by the nematode Strongyloides stercoralis. unique among parasites in that it can remain undetected for decades. Although patients may present with typical symptoms of skin rash, cough or diarrhoea, rare manifestation infection pseudopolyposis colon. This case highlights finding patient strongyloidiasis negative stool studies. Case description: We an 81-year-old man delayed diagnosis strongyloidiasis. The initially presented to clinic evaluation productive and was treated empirically bronchitis. He subsequently developed vomiting diarrhoea imaging revealing circumferential wall thickening ascending colon, consistent colitis. tests were negative, antibiotics, then underwent colonoscopy which revealed multiple polyps. Biopsy results confirmed infection. ivermectin improved. Discussion: describes colonic infected rural southwest Virginia. often misdiagnosed due its non-specific gastrointestinal chronic nature disease. studies are routinely parasitic infection, CT scan findings suggestive colitis should be followed biopsy any abnormal lesion right clinical context. Determining definitive timely manner crucial establishing specific treatment resolution
