A Rare Case of Headache in a Patient With McCune-Albright Syndrome: A Triple Threat

A 26-year-old male presented with facial asymmetry since 11 years of age and painless progressive diminution vision in the left eye 16 age. He an exacerbation headaches for past two months. On examination, he was tall had acral enlargement, craniofacial deformity, bilateral asymmetric testicular enlargement. Investigations revealed high insulin-like growth factor 1, non-suppressible hormone on oral glucose tolerance tests, multiple pituitary deficiencies. MRI showed macroadenoma sphenoid fibrous dysplasia as well tuberculomas. Cerebrospinal fluid testing protein, low glucose, adenosine deaminase, all consistent a diagnosis central nervous system (CNS) tuberculosis. His headache did not respond significantly to either octreotide or zoledronic acid. The patient then initiated antitubercular therapy, which led near-complete resolution CNS lesions within three months therapy. tuberculosis masquerader index case acrogigantism due McCune-Albright syndrome. Headaches may be multifactorial given acromegaly, investigating alternative additional causes especially when dealing treatment-refractory cases can rewarding.
