A Prospective Study on Neonatal Sepsis in a Tertiary Hospital, Nepal

About 20 % of neonates develop sepsis and among them approximately 1% die due to sepsis-related causes. Bacterial pathogens are the commonest cause neonatal which is either early-onset (<72 hours age) or late-onset (>72 hours). Little known about epidemiology antimicrobial susceptibility pattern causing bacterial in Nepal. A prospective study was carried out suspected have admitted Tribhuwan University Teaching Hospital from January December 2016. Clinical suspicion made based on clinical findings laboratory parameters, later confirmed by isolation organisms blood culture. Drug resistance Gram-positive Gram-negative bacteria were studied standard methods. Meropenem resistant processed for detection β-lactamases genes detected X-pert Carba-R (Cepheid) Assays. Of 372 with clinically sepsis, 132 (35.4%) had culture positivity, 47% 53% sepsis. Coagulase-negative Staphylococcus aureus (CONS) most common (37.9%) etiological agent followed Klebsiella pneumoniae (12.9%). all isolates, 81 (61.3%) 22 (27.2%) multi-drug (MDR), three (3.7%) methicillin-resistant S. (MRSA), 14 (17.2%) CoNS; 50 (37.8%) 26 (52%) MDR 29 (58%) β-lactamases. The blaKPC gene four isolates K. pneumoniae, two E. coli, one ABC (Acinetobacter baumanii complex), Enterobacter aerogenes whereas blaNDM isolate Pseudomonas aeruginosa, Acinetobacter complex, aerogenes. Overall mortality causes 7.6% (10 132). One-third cases positive. Late-onset more than early onset. CoNS predominant high rates resistance.
