A Novel Passive Implantable Differential Mechanism to Restore Individuated Finger Flexion during Grasping following Tendon Transfer Surgery: A Pilot Study

چکیده
Tendon transfer surgery is often used to restore hand grasp function following high median-ulnar nerve palsy. This typically reroutes and sutures the tendon of extensor carpi radialis longus (ECRL) muscle all four flexor digitorum profundus (FDP) tendons hand, coupling them together. makes it difficult irregularly shaped objects. We propose inserting a novel implantable passive device between FDP surgically construct differential mechanism, enabling fingers individually adapt irregular contours during grasping. These implants with no moving parts are fabricated from biocompatible materials. tested implants’ ability create flexion index middle when actuated by single in two human cadaver hands using computerized closed-loop control paradigm. In these cadaveric models, enabled significantly more for wide range donor tensions. The also redistributed fingertip forces fingers. When grasping uneven objects, difference contact reduced nearly 23% compared current suture-based surgery. results suggest that self-adaptive possible transfers drive multiple distal tendons.

Implanted passive engineering mechanism improves hand function after tendon transfer surgery: a cadaver-based study.
PURPOSE The purpose of this study was to investigate if a new tendon transfer surgical procedure that uses an implanted passive engineering mechanism for attaching multiple tendons to a single donor muscle in place of directly suturing the tendons to the muscle improves hand function in physical interaction tasks such as grasping. METHODS The tendon transfer surgery for high median ulnar pals...
متن کاملTailoring Tendon Transfer Surgery and Rehabilitation for a Musician: A Case Study
Tendon transfers in hand patients are a commonly performed procedure after extensor tendon rupture. However, the standard side to side technique is not applicable in every patient. We present a case of a musician with unique demands to demonstrate the option to customize surgical technique and therapy regimen to the unique needs of each patient. An extensor indicis proprius to extensor digit...
متن کاملA Comparative Study on Tendon Transfer Surgery in Patients with Radial Nerve Palsy
BACKGROUND Lesions in peripheral nerves are highly prevalent in the upper extremity. The present study compares different tendon transfer surgeries in patients with radial nerve palsy. METHODS Fifty patients with radial nerve palsy were randomly selected among patients who referred to Tehran 15th Khordad Hospital during 2006-2011. They were divided into two groups of 17 and 33 subjects. Sing...
متن کاملEffects of tendon transfer to restore index finger abduction for severe cubital tunnel syndrome
BACKGROUND Severe cases of cubital tunnel syndrome do not always result in functional recovery after surgical decompression of the ulnar nerve. A combined operation of tendon transfer to restore index finger abduction and decompression of the ulnar nerve was performed for patients with severe cubital tunnel syndrome who required powerful pinch strength and whose preoperative compound muscle act...
متن کاملFinger flexor motor control patterns during active flexion: an in vivo tendon force study.
An in vivo tendon force measurement system was used to evaluate index finger flexor motor control patterns during active finger flexion. During open carpal tunnel release surgery (N=12) the flexor digitorum profundus (FDP) and flexor digitorum superficilias (FDS) tendons were instrumented with buckle force transducers and participants performed finger flexion at two different wrist angles (0 de...
