A model of culturally?informed integrated diabetes education and eye screening in indigenous primary care services and specialist diabetes clinics: Study protocol

To improve diabetes management in Indigenous Australians using an integrated nurse-led model of education and eye screening indigenous primary care specialist clinics. A pre–post study. This study will be implemented clinics Victoria, Australia. Participants recruited to the existing adult patient with diagnosed attending sites. nurse-credentialled educator certified retinal imager deliver three components: (a) photography as a diabetic retinopathy engagement tool; (b) lifestyle behaviour surveys, administered at baseline final visit, 12 months. Findings from surveys participants’ images used guide; (c) personalized education. The outcomes are participant adherence recommendations health service coverage. Secondary DR prevalence changes clinical risk factor levels, knowledge satisfaction care. Compared non-indigenous Australians, have high blindness, low suboptimal literacy.
