Abstract Background Achilles tendinopathy (AT) is a common overuse injury in running-related sports where patients experience pain and impaired function which can persist. A graded rehabilitation program has been successful reducing improving to enable return sport. The aim of this study compare the effectiveness criteria-based including strength reactive targets, with previously on changes using Victorian Institute Sport Assessment-Achilles (VISA-A) questionnaire. Secondary aims will be assess calf strength, lower limb running forward hop biomechanics over course 12-week program, long-term follow-up investigations. Methods Sixty eligible participants chronic mid-portion AT who train running-based included study. They randomly assigned group that follow an evidence-based daily exercises progression guided by symptoms or performing 3 high-intensity sessions per week individualised load targets progressing exercises. Testing take place at baseline, 6 12. Plantar flexor peak torque measured isokinetic dynamometry, drop jump biomechanical variables during single leg hurdle test treadmill 3D motion analysis.
