A Clinical Study to Assess the Efficacy of Anterior Transarticular C1-C2 Screw Fixation for Stabilization of AtlantoAxial Instability

Introduction: The upper cervical spine includes the atlas (C1) and Axis (C2). anatomy of twovertebrae is unique from each other. Atlanto-axial articulation most unique, mobile segment spine,which largely depends on ligamentous supports based integrity odontoid for its stability. Historically,atlanto-axial subluxation was treated by reduction fusion C1-C2 joint. However, High riding vertebralartery precludes placement posterior trans-articular screw, which liable to injury during screwplacement. Socomputed tomography scans can be used evaluate risk “high riding” vertebral arteryduring management atlanto-axial subluxation. Hence, anterior screw fixation techniqueavoids course high artery.Material Method: Nine patients underwent NCCT with 3-D reconstruction clinicalevaluation pain done recording VAS score pre operatively after surgery.Results: All surgical cohorts an average follow up 24 months.All were assessed as per fixed protocol our local hospital guidelines orthopaedics department.Pain clinically evaluated before surgery yielding 90% excellent result.Conclusion: Anterior Trans-articular isa minimally invasive technique less bloodloss, shorter skins car faster post-operative recovery. It appropriate stabilization ofAtlanto-Axial instabilities.
