A 62-year-old man with a purpuric rash and acute kidney injury

Staphylococcus aureus bacteremia can infrequently present as vasculitis or Acute Glomerulonephritis (AGN). The association between and these immune-mediated responses is rare remains a diagnostic challenge. We case of 62-year-old man with hypertension, hyperlipidemia, type II diabetes mellitus who presented joint pain, hematuria purpuric rash in his legs oral cavity after he dropped bag heavy metal on right foot. He was found to have acute glomerulonephritis rapidly progressing renal failure requiring emergent hemodialysis. At first, the presentation seemed be rheumatologic origin initially managed methylprednisolone. However, further work-up revealed methicillin-sensitive bacteremia. toe amputated for source control patient treated antibiotics. This is, our knowledge, first reported clinical manifesting rash, glomerulonephritis, pain. It highlights importance making an initial differential diagnosis infectious disorder, suspicion would change management prognosis. Keywords: Bacteremia; Vasculitis; Glomerulonephritis; aureus.
