805 Eluting Electrodes for Hearing Preservation – the Future of Cochlear Implantation?

چکیده
Abstract Introduction Cochlear implants (CIs) utilising eluting electrodes are a relatively new phenomenon. CI surgery can lead to variety of complications, namely electrode insertion trauma. This affects function leading loss residual hearing. To minimise this, anti-inflammatory or growth-promoting agents be used protect and enhance hearing through the sustained release an agent. Eluting include glucocorticoids, steroids, growth factors neurotrophins. Method Systematic review narrative synthesis. Databases searched MEDLINE, EMBASE, CENTRAL, ClinicalTrials.gov Web Science. Results Searches identified 66 abstracts 37 full texts. Of these, 15 studies met inclusion criteria, reporting outcomes in 469 animals 24 humans, with at least 233 implants. included dexamethasone (n = 13), factor 1) neurotrophins 1). Audiological improved across all following cochlear implantation greater extent when compared standard electrode. Outcomes significantly higher concentration was used. Conclusions generally superior omission medication implantation. An elution enables sustained, localised anti-apoptotic agents, therefore reducing advent fibrosis, scarring subsequent impedance.

Methods of Hearing Preservation during Cochlear Implantation
Introduction Recent advances in surgical techniques and electrode design have made residual hearing preservation during cochlear implantation (CI) possible, achievable, and desirable. Objectives The objective of this study was to review the literature regarding methods used for hearing preservation during CI surgery. Data Synthesis We performed a search in the LILACS, MEDLINE, SciELO, PubMed...
متن کاملHearing Preservation after Cochlear Implantation: UNICAMP Outcomes
Background. Electric-acoustic stimulation (EAS) is an excellent choice for people with residual hearing in low frequencies but not high frequencies and who derive insufficient benefit from hearing aids. For EAS to be effective, subjects' residual hearing must be preserved during cochlear implant (CI) surgery. Methods. We implanted 6 subjects with a CI. We used a special surgical technique and a...
متن کاملFactors affecting residual hearing preservation in cochlear implantation
The likelihood of residual hearing preservation in cochlear implantation (CI) is related to surgical factors such as type of cochleostomy (trans-fenestral vs. promontorial), use of lubricants and protective drugs, and device-related factors such as shape, length and flexibility of the array. We investigated the impact of these factors on the hearing preservation rate in adults and children with...
متن کاملThe role of preoperative, intratympanic glucocorticoids for hearing preservation in cochlear implantation: a prospective clinical study.
BACKGROUND Hearing Preservation is becoming increasingly important in cochlear implantation as there is growing evidence that preserving the residual hearing, especially in the low frequencies in combination with the electric stimulation can significantly improve hearing and speech outcomes in noise. Besides the ongoing development of atraumatic implant electrodes and insertion techniques, the ...
متن کاملElectrode selection for hearing preservation in cochlear implantation: A review of the evidence
Objective To review and assess the ideal length of electrode in cochlear implant patients for hearing preservation. Methods The English language literature was reviewed for studies including hearing preservation and speech understanding for electrodes of different lengths. Results One prospective trial was found, and there were no studies that randomized patients into different length elect...
