چکیده

Abstract Background Secondary hyperparathyroidism commonly results from vitamin D deficiency and can lead to accelerated bone turnover loss, especially at cortical sites like the hip. It also attenuate response antiresorptive treatments used for osteoporosis. However, several factors may influence PTH response. We aimed identify prevalence of secondary by categories status in older Irish adults attending a health clinic. Methods The study population consisted (aged over 60 years) clinic large hospital. Participants with serum calcium >2.5 mmol/l eGFR <30 ml/min were excluded avoid cases primary or elevated due advanced renal disease. Hyperparathyroidism was defined as > 65 pg/ml. 25 hydroxyvitamin (25(OH)D) measured liquid chromatography mass spectroscopy. Results There 800 identified, mean age 72.9 +/- 7.9 years, 85.3% female. 25(OH)D 28.1% (<30nmol/l), 17.4% (30-49.9 nmol/l) 8.0% (50-74 nmol/l). Older (P < 0.03) lower = 0.01) associated independent status. Conclusion Nearly one-third patients who deficient (<30 nmol/L) one-sixth insufficient had hyperparathyroidism, similar other studies. observed 8% those levels between 50 -74 nmol/l, suggesting that higher i.e. ≥ 75 nmol/l be preferable adults. Lower independently PTH, consistent previous research. Higher dietary supplemental intake is known suppress response, though we not able account this our study.

Vitamin D analogues for secondary hyperparathyroidism.

Secondary hyperparathyroidism (2HPT), a common disorder in patients with chronic renal failure, develops in response to phosphate retention and low serum 1,25-dihydroxyvitamin D(3) (1,25(OH)(2)D(3), calcitriol). Replacement therapy with calcitriol or its precursor 1alpha-hydroxyvitamin D(3) (1alphaOHD(3), alfacalcidol) often produces hypercalcaemia, especially when combined with calcium-based p...
