53 A Rare Malignant Lesion of the Jaws

Abstract Introduction Primary intraosseous carcinoma (PIOC) of the jaw is an incredibly rare lesion with approximately 200 cases reported in literature. PIOC are mostly asymptomatic incidental radiographic findings and present as poorly-defined, non-corticated radiolucencies often root resorption cortical perforation. Prognosis considered poor best predicted by histological grade. Case report A gentleman his 50s presented to dentist a draining sinus from LR6 tooth. radiograph revealed radiolucency localised distal LR6. 7 months later, he returned pain mobility LR7. Another around extending LR8. Urgent biopsy mandibular bone was arranged oral maxillofacial surgery team. Histopathology likely benign squamous odontogenic tumour. However, it not possible exclude well-differentiated cell carcinoma. Correlation clinical findings, imaging multidisciplinary team discussion recommended. Following MDT meeting, excision bony undertaken. this time primary NOS T4N0M0, thought have arisen radicular cyst associated LR6/7. The treatment right segmental mandibulectomy, fibular free flap, selective neck dissection (I-III) adjuvant radiotherapy. Conclusions This case highlights how subtle features can represent malignancy lesions appearing cystic at first. Although rare, clinicians should be aware differential diagnosis when which could malignancy.
