519 Racial/ethnic differences in delivery gestational age and morbidity among women with hypertensive disorders of pregnancy

We sought to determine whether there are differences between non-Hispanic Black (NHB) and White (NHW) women in delivery gestational age disease severity (maternal neonatal morbidity) among with hypertensive (HTN) disorders of pregnancy. This was a retrospective cohort all self-identified NHB NHW pregnant carrying non-anomalous singleton or twin gestations at tertiary healthcare system, 2014-2020, who were diagnosed HTN disorder pregnancy (gestational HTN, preeclampsia, eclampsia). The primary outcome <37 wks. Secondary outcomes <34 & <28 wks, maternal morbidities [HELLP syndrome, acute renal insufficiency (serum Cr ≥ 1.0), severe range intrapartum BP, morbidity (SMM, per CDC definitions)], composite (diagnosis IVH, PVL, BPD, NEC, ROP, death prior initial hospital discharge). Data analyzed using chi2, t-test, logistic regression. 3,522 met inclusion criteria; 842 (23.4%) delivered 29.8% NHB. Population characteristics shown Table 1. more likely deliver (29.5% vs. 21.5%), (13.7% 6.3%), (2.9% 1.2%) wks compared (all p<0.001). also than have (4.5% 1.9%), systolic diastolic BP (Table 1), SMM (5.3% 2.5%), p<0.001. HELLP syndrome did not differ by race.
