چکیده
Abstract Aim In the absence of metastases, pancreatoduodenectomy (PD) is indicated in fit patients with a resectable pancreatic head/periampullary cancer. Surgeons should have sound understanding its complication profile for consenting purposes and to benchmark their rates. This study aimed calculate overall incidence PD complications identify risk factors. Method A multicentre retrospective cohort confirmed malignancy was conducted (29 centres from 8 countries). The severity all recorded calculated. Patients who experienced any complication, major morbidity, clinically relevant postoperative fistula (CR-POPF), post-pancreatectomy haemorrhage (PPH) 90-day mortality were compared those did not experience these outcomes. Results 1348 included patients, morbidity 53% 4%, respectively. total, 1340 recorded; 72%, 18%, 6% 4% Clavien-Dindo (CD) grades I-II, III, IV V, CR-POPF, PPH, chyle leak, bile leak gastro-jejunal affected 8%, 6%, 3% 2% Following multivariable analysis, high body mass index (BMI, p = 0.007), American Society Anesthesiologists (ASA) grade >II (p<0.0001) pylorus-resecting approach (p 0.005) correlated morbidity. addition, ASA at increased raised BMI CR-POPF 0.001). Conclusions our study, over 50% However, most CD I-II. High BMI, associated studied adverse

Factors predicting adequate lymph node yield in patients undergoing pancreatoduodenectomy for malignancy
BACKGROUND Most pancreatoduodenectomy resections do not meet the minimum of 12 lymph nodes recommended by the American Joint Committee on Cancer for accurate staging of periampullary malignancies. The purpose of this study was to investigate factors affecting the likelihood of adequate nodal yield in pancreatoduodenectomy specimens subject to routine pathological assessment. METHODS Six hundr...
متن کاملAnesthesia for leg amputation in high risk patients: Case reports
Background: High risk cases of leg amputation were one of major concerns in anesthesiologists. Cases: Sixteen cases with American Society of Anesthesiologists physical status (ASA-PS) 3 or 4 for leg amputation were reported. Fifteen patients were medicated with more than two anticoagulants and 11 patients had bleeding tendency. Thirteen patients received below knee amputation (BKA) and thre...
متن کاملExtended Venous Thromboembolism Prophylaxis for High-Risk Patients Undergoing Surgery for Malignancy
BACKGROUND In surgical patients with known malignancy, the odds ratio for an episode of a venous thromboembolism is approximately 6.5 compared to a group of patients without malignancy undergoing the same procedure [Heit et al.: Arch Intern Med 2000;160:809-815]. CASE REPORT We present a case of a 46-year-old Caucasian male with a history of adenocarcinoma of the rectum. The patient received ...

OBJECTIVE To evaluate the feasibility of sentinel node staging for detection of occult regional lymph node metastasis in high-risk cutaneous nonmelanoma malignancies. DESIGN Consecutive clinical case series. SETTING Referral university medical center. PATIENTS A consecutive sample of patients with a variety of high-risk nonmelanoma cutaneous malignancies without evidence of regional lymph...
متن کاملLaparoscopic liver resection for malignancy in high-risk surgical patients according to ASA classification.
Laparoscopic liver resection (LLR) represents one of the most recent evolutions in the field of surgical oncology. While offering to the patients all the short-term advantages of the laparoscopic approach, the ongoing experience underlines that the long-term outcomes are not negatively influenced through this minimally invasive method. We explored the surgical results in a case series of 5 high...
