0797 Reclaiming Her Life: Successful Treatment of Kleine-Levin Syndrome with Transdermal Flumazenil

Abstract Introduction Kleine-Levin Syndrome (KLS) is a rare disorder of hypersomnolence with cognitive and behavioral disturbances. Currently, there are no definitive treatment recommendations although stimulants, mood stabilizers, even electroconvulsive therapy have been utilized inconsistent results. Report Cases: A 23-year-old female history depression, anxiety, obsessive-compulsive disorder, Hashimoto's thyroiditis, lupus, morbid obesity presented to our facility for management debilitating episodes sleepiness due KLS, diagnosed at the age 22. She reported good health until series viral illnesses 20. then began experiencing sleep requiring 20-22 hours per day, only awakening eat use bathroom. These typically lasted two weeks were preceded by periods hyperphagia hypersexuality. When she facility, was using lithium modafinil daily along amantadine clarithromycin twice as needed sleepiness. At her initial visit, prescribed transdermal flumazenil, GABA-A receptor/benzodiazepine antagonist, be used supplement clarithromycin. initially flumazenil every other month. After seven months use, frequency decreased 3-4 months. would administer clicks cream on each forearm prodrome symptom fogginess, which abort episode. Rarely, required second dose 12 later. With avoidance triggers, such alcohol deprivation, completing master’s degree finally maintaining job. Ultimately, discontinued lithium, modafinil, amantadine, without reoccurrence symptoms. Conclusion This case demonstrates successful KLS flumazenil. Reduction in ultimately led discontinuing stimulant, stabilizer, use.
