077 Cicatricial bullous pemphigoid and sjogren's syndrome: Case report in an elderly female

This case discusses the management of an 83-year-old female patient from Lebanon with Sjogren's Syndrome and Cicatricial Bullous Pemphigoid. The presented in January 2021 to internal medicine clinic for evaluation on off pruritic, burning blisters lower abdomen pubic area. She has hypothyroidism a 40 year long history difficulty exhaling through her nose. Family social unremarkable except using 2-5 cigarettes per day years. Her groin symptoms started spring 2019 by June 2020 developed sporadic pruritic non-pruritic rashes other locations progressive eye tearing redness. Upon primary care visit, she was being treated antibiotic steroid drops. taking oral antibiotics intermittently before U.S. began. labs were consistent staph aureus COPD so prescribed doxycycline later topical steroids, but lesions persisted. In consulted ENT increased breathing nose, who referred rheumatology because inflammation seen scope. diagnosed Sjorgen’s May (positive SS-A Abs speckled ANA) hydroxychloroquine 200mg plus prednisone 10mg daily. ophthalmology March redness watering informed that makes it impossible open tear ducts both eyes. same dermatologist suspected cicatricial bullous pemphigoid, given ocular dermatologic findings. A positive biopsy prompted switching HCQ 6-mercaptopurine 25mg then 50mg. patient's resolved quickly presence allergic type rash elevated liver enzymes soon afterwards daughter stop 6-MCP while continuing steroids. currently denies skin nose complaints remain. recommends rituximab cyclophosphamide, not them possible side effects.
