We report the case of a 35-year-old man presenting with a delayed and prolonged coma due to an intentional overdose with disulfiram without simultaneous alcohol ingestion. The clinical features--comprising a severe toxic encephalopathy with coma and convulsions, in combination with a quadriparesis outlasting the loss of consciousness--are summarized, and the physiopathology is reviewed.

A patient with fulminant Guillain-Barré syndrome developed pupillary denervation, loss of all brain stem reflexes and a complete flaccid quadriparesis. Pathological studies confirmed this was due to a primarily demyelinating process and not an axonal form of Guillain-Barré syndrome.

We report a case of distal renal tubular acidosis in a twenty year old female patient of scleroderma and secondary Sjogren's syndrome. This patient presented with two episodes of flaccid quadriparesis which were associated with hypokalaemia and was later found to have an underlying scleroderma with secondary Sjogren's syndrome.

Neonatal hyperekplexia is a rare autosomal dominant startle disorder. Presenting soon after birth, it is often mistakenly diagnosed as spastic quadriparesis, epilepsy etc. While the long-term prognosis is relatively benign, sudden death due to severe spasms have been seen in sporadic cases. We report a case of hyperekplexia with some typical and some unusual findings.
