This paper describes current treatment strategies of distal femoral fractures as well as their evidence based rationale. The treatment of distal femoral fractures has improved with the evolution of plating and nailing technologies. The commonly selected surgical approaches are outlined and surgical treatment techniques including both internal and external fixation are discussed.

Closed intramedullary nailing of the femur is a complex technique but one that has considerable advantages. The choice of an appropriate point of entry into the proximal femur during introduction is vital, since an oblique insertion of the nail may lead to impaction of the implant and comminution of the proximal femoral fragment (Winquist, Hansen and Clawson 1984); this is particularly so in sm...

Ultrasound imaging used to facilitate performance of a femoral nerve block also affords imaging of adjacent anatomical structures. Following a fracture of the femur, an ultrasound guided femoral nerve block (UGFNB) was performed to provide analgesia; this led to the incidental finding of a previously undiagnosed femoral vein thrombosis (DVT), resulting in a change in patient management before s...

OBJECTIVE To describe the applications and effects of electromagnetic navigation (EN) technology in distal locking for the treatment of long diaphyseal fracture (LDF) with interlocking intramedullary nailing (IIN). STUDY DESIGN An interventional study. PLACE AND DURATION OF STUDY The Second Affiliated Hospital of Soochow University, China, from March 2013 to July 2014. METHODOLOGY Patient...

BACKGROUND There are several different techniques commonly used to perform intramedullary (IM) nailing of the femur to fix femoral fractures. We sought to identify significant differences in outcomes of studies comparing 1) trochanteric and piriformis entry and 2) antegrade and retrograde entry in IM nailing of the femur. METHODS We searched MEDLINE, Cochrane and Embase databases and the Orth...

Abstract Background: Femoral malrotation is one of the most common complications following closed intramedullary nailing, (IMN). We hypothesized that the patients’ position during surgery can affect the incidence and severity of this complication. The aim of the study was to compare the incidence and severity of femoral malrotation in patients with femoral diaphyseal fractures treated with IMN...

PURPOSE To review the outcome of retrograde locked nailing for aseptic supracondylar femoral nonunions following failed locked plating. METHODS Records of 20 men and 4 women aged 20 to 57 (mean, 39) years who underwent retrograde locked intramedullary nailing for aseptic supracondylar femoral nonunion following failed locked plating were reviewed. There were 20 closed and 4 open Gustilo type ...

Background:Femoral shaft fractures account for 1.6% of all pediatric bony injuries..Fixation of femur fractures in children by ender's nailing is becoming widely accepted because of the lower chance of iatrogenic infection and cost .The objective of the study was to study the functional outcome, duration of union and the complications following the use of ender's nail for femoral shaft fractures...

BACKGROUND Intramedullary nailing is the accepted form of treatment for impending or pathologic fractures of the femoral diaphysis. Traditional teaching promotes the use of a cephalomedullary nail so that stabilization is provided for the femoral neck in the event that a future femoral neck metastasis develops. However, that approach may add cost, surgical time, blood loss, and added radiation ...

The perioperative imaging of a pregnant woman presents a unique challenge to orthopaedics surgeon as concern about the radiation risk to the embryo or fetus. This article highlights effect of radiation exposure on foetus.I would like to discuss about the amount of radiation exposure in a pregnant woman during fluoroscopic guided closed femoral Interlock nailing as radiation has significant...
