The established management of epiphrenic diverticula combines diverticulectomy with myotomy of the distal esophagus including the lower esophageal sphincter. We report a patient with prompt recurrence of an epiphrenic diverticulum after laparoscopic transhiatal diverticulectomy associated with esophagomyotomy and partial posterior fundoplication due to an incomplete myotomy.
We report a 38-year-old man with intestinal obstruction following transhiatal esophagectomy for carcinoma esophagus; it occurred secondary to herniation of the transverse colon through the esophageal hiatus into the mediastinum. The patient is asymptomatic after reduction of the hernia and repair of the disphragmatic hernia.
BACKGROUND Esophagectomy is considered to be the main treatment for esophageal malignancies. Among the different methods, transhiatal approach can cause less damage to patients and is widely used; however, manipulation of esophagus result in heart rate and blood pressure variations. The aim of this study was to investigate changes in blood pressure and heart rhythm during transhiatal esophagect...
TRANSTHORACIC VERSUS TRANSHIATAL ESOPHAGECTOMY: COMPARATIVE STUDY REGARDING SURGICAL APPROACH IN ESOPHAGEAL CANCER (Abstract): BACKGROUND: Surgical resection can offer the best curative treatment for oesophageal cancer but is associated with high postoperative morbidity rates. Most common surgical approaches are transthoracic (TT) and transhiatal (TH) techniques. Transhiatal approach has the ad...
Esophagectomy is the appropriate treatment for patients with resectable esophageal cancer. In common with conventional surgeries, controversy exists about what is the best approach and extent of the dissection. For many years, the procedure of choice for the incision has been the Ivor Lewis procedure, later modified. The tumor is resected through a right thoracotomy combined with a laparotomy w...
Esophageal cancer invading the muscularis mucosa sometimes involves regional lymph node metastases. However, lymph node metastases are rare in the dorsal area of the thoracic aorta. We describe a patient with an intramucosal esophageal cancer invading the muscularis mucosa, accompanied by lymph node metastases in the dorsal area of the thoracic aorta. These lesions were successfully resected by...

BACKGROUND Traditional esophagectomy may be associated with mortality, considerable morbidity, and lengthy recovery. It is often performed in cancer patients who are typically older, have associated comorbidities, and are often malnourished, all factors that increase surgical risk. Minimally invasive esophagectomy has the potential advantages of being a less traumatic procedure with an easier p...
