Following chronic daily headache, neoplastic meningitis was diagnosed in a non-smoking 44-year-old man. A bronchial adenocarcinoma was found to be the primary tumor. He was treated with erlotinib. Meningitis as the first manifestation of a malignancy is very rare.

Meningitis caused by Pseudallescheria boydii is an uncommon infection of the CNS that usually has a poor prognosis and a difficult treatment. We describe a case of chronic meningitis caused by P. boydii in an immunocompetent host that was successfully treated with voriconazole, a new antifungal agent.

Coccidioidal meningitis is a potentially lethal infection. Disease progression while taking fluconazole is a common complication and safe, effective, alternative treatments are limited. Posaconazole therapy resulted in symptomatic and laboratory improvement in 2 patients and clinical improvement in a third patient with chronic, previously unresponsive coccidioidal meningitis.

The clinical presentations of cryptococcal meningitis in HIV-negative patients may be different from that infected with HIV. We report a case of 75-year old male with chronic lymphoid leukemia presenting with recurrent syncope, bi-frontal headache and diplopia. The case is described of an 18-year-old schoolboy who developed pericarditis during the course of meningococcal meningitis; he had evidence of a preceding chronic meningococcaemia. Good recovery followed on systemic antibiotics only. The pericarditis was probably a direct septic complication of meningococcal meningitis.
