BACKGROUND/AIMS Some patients with restrictive thyroid ophthalmopathy, referred for consideration of extraocular muscle surgery, complained of flashing lights in the superior visual field on upgaze. The frequency was assessed and the pathogenesis of this previously unreported symptom explored. METHODS 30 patients were recruited, all of whom had tight inferior recti and were in the burnt out p...

A 19-year-old boy developed an adduction deficit after bilateral radiofrequency ablation of the inferior turbinate and septoplasty. Postoperative imaging disclosed an intact medial orbital wall and high T2 signal and enlargement of the medial rectus. Within 3 months, the motility deficit had resolved. This manifestation is attributed to injury of the medial rectus from radiofrequency ablation a...

Background: Isolated rectus muscle ruptures are rare ophthalmic emergencies which may result in permanent diplopia. Objective: To highlight updates on clinical features, surgical treatment options and potential complications for isolated inferior rectus ruptures. Case: A 39-year-old man came to our emergency department complaining of diplopia and upward deviation of the right eye after experien...

PURPOSE To introduce a novel technique of inferior rectus recession operation to allow larger amounts of recession without causing lower lid retraction and to compare this method with the results obtained in standard inferior rectus recession. MATERIAL AND METHODS This study included 20 patients operated on in the authors' clinic. The median age of the patients was 24.5±18.6 (4-73) years and ...

INTRODUCTION Injury is a known cause of monocular blindness. Ocular trauma may affect lacrimal canaliculi and the extraocular muscle. We report this case as it includes injury to lid, lacrimal canaliculi and inferior rectus. CASE DESCRIPTION A 25-year-old male presented with an injury caused by a sharp object that resulted in a conjunctival tear, lid tear involving the lacrimal canal, and rup...

Anatomical studies demonstrate selective compartmental innervation of most human extraocular muscles (EOMs), suggesting the potential for differential compartmental control. This was supported by magnetic resonance imaging (MRI) demonstrating differential lateral rectus (LR) compartmental contraction during ocular counterrolling, differential medial rectus (MR) compartmental contraction during ...

BACKGROUND Muscle slippage is an adverse outcome of strabismus surgery. Its incidence is increased if adjustable sutures are used or if surgery is performed on the inferior or medial rectus muscles. Although there are no firm numbers for this complication, studies have suggested incidence rates between 7% and 41% when adjustable suture surgery is performed on the inferior rectus muscle. In theo...

The rectus extraocular muscle (EOM) pulleys constrain EOM paths. During visual fixation with head immobile, actively controlled pulleys are known to maintain positions causing EOM pulling directions to change by one-half the change in eye position. This pulley behavior is consistent with Listing's law (LL) of ocular torsion as observed during fixation, saccades, and pursuit. However, pulley beh...

Rupture of the extraocular muscle in the absence of significant injury to the eyeball and adnexa is uncommon. The authors report a case of blunt trauma of the eyeball associated with an inferior oblique muscle and an inferior rectus muscle rupture. A 55-year-old man slipped and fell down hitting his eye on an extended windshield wiper blade. Although he had treatment in the emergency room, he c...

A 52-year-old woman underwent a secondary cosmetic blepharoplasty for repair of residual dermatochalasis. After this procedure vertical diplopia was noted. Ultrasound examination and the findings at operation were consistent with trauma to the inferior rectus muscle. We present this as an additional complication of cosmetic blepharoplasty.
