Barium peritonitis is a rare but life-threatening complication following gastrointestinal contrast study. We presented the case of patient suffering from intestinal perforation after undergoing small bowel series. Although later computed tomography confirmed diagnosis, prudent interpretation plain X-ray film might expedite diagnosis and reduce utilization medical resources.

The techniques of radiological examination of the small bowel are considered. The importance of the routine barium meal and follow-through examination is stressed.
