膵癌診療ガイドライン（2019年版）1）に，膵癌では手術例数の多い施設で外科的治療を行なうことを提案する，と記載されている。当院はいわゆるhigh volume centerではないが，当科での治療を希望する患者に対して，これまで高難度の肝胆膵外科手術を積極的に行なってきた。2010年～2019年の10年間に施行した膵頭十二指腸切除術78症例の手術関連因子，周術期合併症，長期予後等について検討した。手術関連死亡はゼロであり，膵頭部癌手術症例の5年生存率は31％と比較的良好な結果が得られ，術後成績，長期予後に関して，いわゆるhigh centerの成績と比べて遜色ないものと思われた。地域の中核病院として，今後も継続して安全で質の高い医療を提供し続けなければならないと考えている。
