Miller Fisher syndrome (MFS), a variant of Guillain-Barré syndrome, is a rare disorder typically characterized by a triad of ataxia, areflexia, and ophthalmoplegia, which may have a highly variable clinical presentation. We report a case of MFS in a 45-year-old female presenting with sphenoid sinusitis and sixth nerve palsy. She underwent endoscopic sphenoid sinusotomy without improvement, had ...
Fungal infections of the nose and paranasal sinuses are frequent in this part of the world. One aspect of the increased frequency is the improved clinical, radiological and pathological diagnostic methods that are now available. Aspergillus is the commonest infecting fungus of the nose and the paranasal sinus. It is mainly a saprophytic spore producing fungus and can cause invasive and non-inva...
Pneumocephalus is a rare but important complication of acute isolated sphenoid sinusitis (ISS). If not adequately treated, it may cause serious neurological and life-threatening complications. We report the presence of free air in the cavernous sinus arising from intracranial extension of acute ISS. A 41-year-old healthy man presented to our department with a 7-day history of headache. Neurolog...
A case of isolated sphenoid fungal sinusitis in an elderly diabetic patient is described. A coexisting mass lesion located in the sellar region was detected incidentally. Coincidence of these two entities represents a potential surgical disaster which may result in direct intracranial spread of fungal infection.
Introduction Abscesses of the diencephalon are extremely rare. This case report gives details of a patient with panhypopituitarism due to an abscess of the hypothalamus and pituitary gland. The clinical, endocrinological, and radiological findings are described and and compared with previous case reports. A common pattern of signs and symptoms and a relationship to sphenoid sinusitis can be rec...
A patient with chronic unilateral maxillary sinusitis caused by a foreign body (dental amalgam) and mistaken diagnosis of extensive ethmoid neoplasm is presented. The imitation of tumour symptoms was due to a long presence of foreign body in the maxillary sinus and chronic inflammation of maxillary, ethmoid and sphenoid sinuses.
Lemierre syndrome (LS) typically consists of a septic thrombophlebitis of the internal jugular vein caused by Fusobacterium necrophorum. We present an unusual variant of LS with cavernous sinus thrombosis and occlusion of the ipsilateral internal carotid artery secondary to sphenoid sinusitis caused by Streptococcus viridans.
Hyperprolactinemia is a rare entity in the pediatric population. The most common causes of hyperprolactinemia include drug use, hypothyroidism and renal insufficiency, though rarely a pituitary or sellar mass is discovered. We present an immunocompetent pediatric patient who presented with gynecomastia and was found to have hyperprolactinemia. Imaging showed a sphenoid mass and referral was mad...
We report a case of eosinophilic chronic rhinosinusitis (ECRS) associated with optic neuropathy. The visual acuity in the right eye was suddenly reduced to no light perception on awakening in the morning. Fundus examination of both eyes on the same day showed no remarkable changes. Emergency computed tomography showed pan-sinusitis bilaterally and a partial defect of the sphenoid bone on the ri...
Nasopharyngeal carcinoma (NPC) may spread to the paranasal sinuses. This retrospective study describes the features of paranasal sinus involvement in NPC on computed tomography (CT) and magnetic resonance imaging (MRI). One hundred and fourteen patients with histologically proven NPC underwent staging with both CT and MRI. Maxillary sinus infiltration was demonstrated on MRI in 10 patients; sph...
