BACKGROUND Intrathecal anesthesia is commonly used for cesarean section. Bupivacaine and ropivacaine have all been used as intrathecal drugs. The minimum effective local anesthetic dose (MLAD) of intrathecal ropivacaine for non-obstetric patients has been reported. However, few data are available on the MLAD of hyperbaric ropivacaine for obstetric patients and the relative potency to bupivacain...

BACKGROUND AND OBJECTIVES Hyperbaric bupivacaine is commercially available in its racemic form. In Brazil, 50% enantiomeric excess bupivacaine (S75:R25) was introduced as an isobaric presentation. The objective of this study was to evaluate different volumes of hyperbaric S75:R25 bupivacaine in spinal blocks for infraumbilical surgeries. METHODS Forty patients, ages 20 to 60 years, physical s...

Equal volumes (2.5 ml, 12.5 mg) of plain 0.5% bupivacaine (glucose-free) and hyperbaric 0.5% bupivacaine (in 8% glucose) were compared in a randomized double-blind study of 40 patients undergoing Caesarean section under subarachnoid anaesthesia. There were no differences in the rate of onset, maximum spread, number of patients with high cervical levels, duration of anaesthesia or incidence of p...

In the morning a healthy 40 years old female had a successful spinal anesthetic with bupivacaine 1% 10 mg for hemorrhoidectomy followed by a second injection at midnight for severe pain. The second traumatic injection caused a conus medullaris/cauda equina lesion discovered by MRI 45 days later. The contribution of hyperbaric bupivacaine dosage and pencil point needle is discussed.

PURPOSE To describe a case of complete neurological recovery from cauda equina syndrome lasting ten months following spinal anesthesia with 0.5% hyperbaric bupivacaine and epidural anesthesia with ropivacaine, and to discuss the possible mechanisms involved. CLINICAL FINDINGS A 79-yr-old man with Paget's disease was scheduled for surgery to remove a skin tumour below his scrotum. He had no hi...

Background: Shivering is one of the common problems in spinal anesthesia. The objective of the present study was to evaluate the effect of intrathecal fentanyl (25 μg) on incidence and severity of intraoperative and postoperative shivering. Methods: A double-blind randomized controlled study was conducted in eighty healthy women (ASA Physical status I) scheduled for elective cesarean section u...

The effects of adding 0.2 mg preservative-free morphine sulfate in 0.2 ml solution to hyperbaric spinal bupivacaine were evaluated in a double-blind randomized prospective study of 34 patients undergoing elective repeat cesarean section. In the control patients (n = 17), 0.2 ml saline instead of morphine was added to bupivacaine. The intrathecal morphine significantly improved intra- and postop...

Several studies have reported transient neurological symptoms after spinal anaesthesia with 5% lignocaine. In order to evaluate the role of concentrated solutions of local anaesthetic in the development of transient neurological symptoms, 200 ASA I or II patients undergoing minor orthopaedic or rectal surgery under spinal anaesthesia were allocated randomly to receive 4% mepivacaine 80 mg or hy...

The number of obese parturients seen in the practice of obstetric anesthesia is steadily increasing. Many of these women deliver by cesarean delivery under spinal anesthesia. The effect of weight and body mass index (BMI) on the spread of intrathecally administered local anesthetics is controversial. There are a limited number of studies that have correlated the weight or BMI with the spread of...
