Selective electrophysiological involvement of median motor fibers is rare in carpal tunnel syndrome. We report a patient with compression neuropathy of the median nerve who presented with isolated motor signs and symptoms without macroscopic evidence of compressive pathologies.
INTRoduCTIoN Carpal tunnel syndrome is a most common peripheral compression neuropathy (1). It is caused by mechanical compression of the median nerve as it traverses the carpal tunnel of the wrist. Classic signs and symptoms are numbness of the lateral three digits and weakness of the thenar muscles due to atrophy (2). Important diagnostic tests include electromyography (EMG) and nerve conduct...
When a patient presents with pain or paresthesias of the hand and fingers, knowing what to ask, what to look for, and which tests to consider is essential.
In a series of 70 patients (75 cases of common peroneal nerve palsy) the common causes were trauma about the knee or about the hip, compression, and underlying neuropathy. A few palsies occurred spontaneously for no apparent reason. The prognosis was uniformly good in the compression group; recovery was delayed but usually satisfactory in patients who had suffered stretch injuries. In the acute...
Malignant lymphoma around the orbit is very rare. We present a rare case of optic neuropathy caused by lymphoma. A 61-year-old Japanese woman was referred to our hospital for evaluation of idiopathic optic neuropathy affecting her right eye. The patient was treated with steroid pulse therapy (methyl-predonisolone 1 g daily for 3 days) with a presumed diagnosis of idiopathic optic neuritis. Afte...
Cranial neuropathy is usually idiopathic and familial cases are uncommon. We describe a family with 5 members with cranial neuropathy over 3 generations. All affected patients were women, indicating an X-linked dominant or an autosomal dominant mode of inheritance. Our cases and a review of the literature suggest that familial idiopathic cranial neuropathy is a rare condition which may be relat...
INTRODUCTION Synovial cyst is a rare cause of compression neuropathy and its differential diagnosis can be misleading. PRESENTATION OF CASE This article presents clinical, radiological, and histological findings of deep peroneal nerve palsy due to compression of a synovial cyst in a 30-year-old patient admitted with sudden drop foot. DISCUSSION Focal nerve entrapment in lower extremity due ...
Hereditary neuropathy with liability to pressure palsies (HNPP), an autosomal dominant disorder, is characterized by recurrent isolated nerve palsies, which are precipitated by trivial compression or trauma. In this report, we present the clinical features, electrophysiological studies, nerve biopsy results, and molecular analyses of a Taiwanese family. Among the 7 members evaluated, one latent...
