A patient is described in whom extreme right ventricular hypoplasia and right-to-left shunting through an atrial septal defect occurred after relief of severe pulmonary stenosis. The ability of the hypoplastic right ventricle to deal with an increased volume load was assessed at cardiac catheterisation by occluding the atrial septal defect with a balloon tipped catheter.

Two cases are reported of truncus arteriosus communis arising entirely from the right ventricle and associated with interruption in one case and tubular hypoplasia of the aortic arch in the other. A haemodynamic hypothesis of preferential blood flow is proposed to explain this rare association.

A 2-month-old patient with transposition of the great arteries, a ventricular septal defect, pulmonary stenosis, and severe hypoplasia of the right ventricle successfully underwent a biventricular repair utilizing the aortic translocation technique. Advantages of this surgical repair over the Rastelli procedure in the management of this complex congenital heart lesion are discussed.
