Cardiac stereotactic body radiation therapy (cSBRT) is an innovative and promising noninvasive treatment option for recurrent, therapy-refractory ventricular tachycardia (VT). Although long term efficacy safety still under evaluation in multicenter studies, it associated with a significant short-term reduction of VT burden ICD shocks.1–3 To date cSBRT neither first line strategy ablation nor pr...

A 64-year-old man presented with several weeks of intermittent irregular palpitations. He had no prior history of cardiac disease, hypertension or syncope. A 12-lead ECG revealed sinus rhythm with premature atrial and ventricular contractions and high QRS voltages consistent with LV-hypertrophy. Cardiac MR revealed asymmetrical septal hypertrophy and marked mid-myocardial hyperenhancement of th...

For over a century, the behavior of the aorta and other large arteries has been described as passive elastic tubes in which no active contraction occurs in the smooth muscle wall. In response to pulsatile pressure changes, the vessels undergo a 'passive' elastic dilatation-contraction cycle, described as a "Windkessel" effect. However, Mangel and colleagues have presented evidence that is contr...

Abstract Background: Implantable cardiac defibrillators (ICDs) therapy for primary prevention (PP) of sudden arrest (SCA) is well-established but underutilized globally. The Improve SCA study has identified a cohort patients called 1.5 (1.5PP), based on PP with the presence documented risk factors: non-sustained ventricular tachycardia, frequent premature contractions, left ejection fraction < ...

We noticed an unprecedented sensing error: after a correctly sensed ventricular extrasystole followed by a correctly timed atrial stimulus, a ventricular impulse occurs that is not sensed, then the delivery of a ventricular stimulus can be observed at the set AV delay (225 ms) (Fig. 1). The subsequent stimuli appear to be sensed correctly, judging from the lack of any atrial or ventricular spikes.

AN apparently haphazard distribution of ventricular extrasystoles may not be so haphazard as it appears and indeed may mask an underlying regularity. We previously reported a case in which careful analysis uncovered a simple but remarkable phenomenon, namely, that the interectopic intervals always contained an odd number of sinus beats.1 This was explicable on the assumption that a bigeminal rh...

Eight patients with ventricular extrasystoles are reported in whom coupling intervals of the extrasystoles to the proceding sinus beats were variable and in whom paired ventricular extrasystoles were occasionally seen. In all patients, paired ventricular extrasystoles were initiated only by comparatively late coupled ventricular extrasystoles. However, the interval between the first and the sec...
