A 40-year-old HIV-positive male (CD4 count 228 cells/ul) presented with decreased vision to 20/60 OD and handmotions OS. Funduscopic examination OD revealed 2+ vitritis with widespread intraretinal whitening, fluffy preretinal vitreous opacities, and a subhyaloidal hypopyon (a). A 4+ severe vitritis precluded retinal examination OS. PCR testing of aqueous fluid OD was negative for HSV, VZV, CMV...
We report a positive outcome of postcataract endophthalmitis caused by Enterobacter cloacae, which has previously resulted in poor outcomes in endophthalmitis. A 67-year-old man underwent uncomplicated cataract surgery. On the morning of postoperative day (POD) #1, he had significant anterior chamber inflammation without pain, hypopyon, or vitritis but then rapidly developed hypopyon and worsen...
EDITOR,—Between 16% and 40% of patients with AIDS develop cytomegalovirus retinitis (CMVR). 2 This typically presents when the CD4+ count drops below 75 cells/μl. Visual loss is usually due to retinal detachment, macular or optic nerve infection, or vascular occlusion. Typically a minimal inflammatory response is mounted. Patients with CMVR not associated with HIV infection may develop a vitrit...
A case of anterior internal ophthalmomyiasis is described. A 27-year-old female from Northern Brazil presenting with anterior uveitis and vitritis had a fly larva surgically removed from the anterior chamber of the left eye. The species was Cochliomyia hominivorax. The larva was covered by macrophages and foreign body giant cells characterizing a foreign body granulomatous reaction.
PURPOSE The incidence of infectious endophthalmitis associated with intravitreal injection (IVI) of steroid has been reported to be as high as 0.87%. This study was designed to investigate whether the antimicrobial activity of intravitreal methotrexate (MTX) alters the incidence or course of bacterial endophthalmitis associated with IVI in a rabbit model. METHODS A rabbit model of endophthalm...
Ocular toxoplasmosis frequently presents as necrotizing retinochoroiditis and, less often, peripapillary chorioretinitis and/or papillitis. The progression from papillitis to has been rarely described. We report the case of a 52-year-old patient living in southern Brazil, who developed right eye and was treated with systemic corticosteroids (prednisone 0.6 mg/kg/day pulse therapy methylpredniso...
PURPOSE To test the antiviral effect of ganciclovir released from biodegradable polymer microspheres in rabbit eyes inoculated with human cytomegalovirus (HCMV). METHODS Human cytomegalovirus (5 x 10(3) plaque forming unit in 0.1 ml Hank's balanced salt solution) was inoculated 4 days after gas compression vitrectomy. Injected after 2 days was 10 mg of 300- to 500-micron ganciclovir-loaded mi...
OBJECTIVE To compare distinctive clinical features of presumed tuberculous serpiginouslike choroiditis (Tb-SLC) with classic serpiginous choroiditis (SC) in patients living in a region that is nonendemic for tuberculosis. METHODS Retrospective comparative analysis of clinical features of 5 patients with recurrent Tb-SLC and 5 with SC. RESULTS All patients with recurrent Tb-SLC primarily emi...
