症例は77歳女性. 3年来増悪する頭痛が主訴であった. 右奥歯が締め付けられる感覚と分泌低下を示唆する口腔内乾燥が認められた. 頭部造影MRIで右翼口蓋窩～右側頭窩に囊胞性の腫瘍が指摘された. 経鼻的な前篩骨洞方向へのアプローチに加え, 上顎洞経由の犬歯窩アプローチにて腫瘍亜全摘を行った. 術後, 口腔内の分泌低下は残存したが, 頭痛や奥歯の違和感は改善し, 新たな神経症状も認められず, 経過良好であった. 病理組織診断は神経鞘腫であり, 発生母地からvidian nerve schwannomaと推測された. 翼口蓋窩腫瘍に対する内視鏡を用いたアプローチは侵襲が小さく治療戦略として有用であると考えられた.
