The accessory palmaris longus is a rare anatomical variant in the wrist and forearm, which has been reported as cause of carpal tunnel syndrome. This paper reports case incidentally found during surgery. paucity awareness on surgery may lead to accidental iatrogenic injury median nerve or insufficient decompression nerve.

A 4 1-year-old man presented with an occult median nerve injury. This report highlights the importance of adequate wound exploration and reinforces the significance of adequate history and examination in patients with signs of nerve injury.
