症例は80代，女性．排便後に出現した腹痛と下腹部の異常を主訴に受診した．診察時，肛門部に小腸が脱出しているという特異的な形態異常を示した．直腸穿孔に伴う小腸脱出と診断し緊急手術を施行した．直腸脱の手術歴があり結腸が骨盤底に癒着していたが，直腸穿孔箇所を同定し穿孔部縫合閉鎖と人工肛門造設術を施行した．退院後に人工肛門管理の不自由さがあり人工肛門閉鎖術を行った．その後再発はない．小腸脱出を伴う特発性直腸穿孔の報告例は非常に稀である．排便時などの腹腔内圧上昇を契機とし，直腸脱や子宮脱などの手術歴のある高齢者の報告例が散見される．特異的形態異常を示すため比較的早期に緊急手術を施行され予後も良いが，術式選択に難渋することがある．今回，小腸脱出を伴う直腸穿孔という稀な症例に対し，年齢・PS・患者背景・手術の安全性などに配慮しつつADL維持を図りながら外科的治療し得たので症例報告する．
