Serious ventricular arrhythmias are known to occur in patients with long QT intervals. We describe a case of torsade de pointes occurring in a patient with a prolonged QT interval while taking a 1000 calorie diet, diethylpropion hydrochloride (Tenuate Dospan) and bendrofluazide. In patients with long QT intervals, hypokalaemia and drugs which further delay repolarization may facilitate the deve...
A retrospective analysis of routinely collected data from a diarrhoea rehydration unit found clinically meaningful parallel seasonal variation in plasma sodium and potassium concentrations. The prevalence of severe hypokalaemia was 7.2% and 0.4% in February and August respectively, and of severe hypernatraemia 0.4% and 5.0% respectively. These unexpected findings need prospective confirmation a...
Hyper-reninaemia, hypokaluria, and hypokalaemia in an infant with congenital chloride diarrhoea improved during treatment with a prostaglandin synthetase inhibitor, ketoprofen. There was evidence of increased activity of therenin-aldosterone system when ketoprofen was stopped. It is suggested that prostaglandins may be involved in stimulating the renin-aldosterone system in congenital chloride ...
Arterial hypertension is a highly prevalent disease and its secondary causes must always be kept in mind because the treatment and prognosis differ between these and essential hypertension. Here we present the first reported case in Argentina of a 21-year-old patient with arterial hypertension and hypokalaemia due to a renin-secreting juxtaglomerular cell tumour, which was diagnosed after seven...
Hyperventilation reduces partial pressure of CO2 (PCO2) in the blood, which results in hypokalaemia. Hypokalaemia helps the development of the life-threatening torsades de pointes type ventricular arrhythmia (TdP) evoked by repolarization delaying drugs. This implies that hyperventilation may assist the development of proarrhythmic events. Therefore, this study experimentally investigated the e...
Nurofen Plus is a common analgesic containing ibuprofen and codeine. We present a case of a 38-year-old lady who developed renal tubular acidosis with severe hypokalaemia, after chronic abuse of Nurofen Plus tablets. She presented with confusion and profound biochemical abnormalities requiring critical care admission for electrolyte replacement. Ibuprofen causes renal tubular acidosis due to it...
BACKGROUND/AIM Reninomas are rare juxtaglomerular tumours which can cause severe hypertension and hypokalaemia. Diagnosis can be problematic and these tumours can be difficult to locate on imaging. In this report we aim to demonstrate the value of carefully performed renal vein renin ratios (RVRRs) to assist in locating these tumours. METHOD/RESULTS We report on 3 patients diagnosed with reni...
Six children are described in whom deafness followed treatment of full thickness burns with topical antibiotic spray containing neomycin. 3 children developed hypocalcaemic tetany, and were shown to have a metabolic disorder involving hypocalcaemia, hypomagnesaemia, and hypokalaemia. The dangers of treating burns with topical ototoxic and nephrotoxic antibiotics are emphasised.
