症例は47歳,女性.5日前から心窩部痛が出現したため,当院へ来院.同部に圧痛を認めたが発熱なく,血液検査でもCRP軽度上昇以外は異常なかった.画像検査で肝円索部に厚い被膜を有する45mm大の嚢胞あり.感染性嚢胞を疑い抗菌薬を投与したところ,腹痛は数日で消失した.肝円索膿瘍の術前診断で手術を施行したところ,肝円索に5cm大の腫瘤が認められ,約1.5cmにわたって肝表面に接していた.摘出標本は55×33mm大の単房性嚢胞で,内腔には乳白色調の液体が貯留しており,病理組織学的に肝円索との連続性はなかった.嚢胞壁の内面は線毛を有する多列円柱上皮で被覆され,上皮下は疎性結合織,平滑筋層を有する線維性結合織からなり,線毛性前腸性肝嚢胞と診断された.自験例は,その発育が通常例のように肝被膜直下つまり内方向ではなく,肝円索への外方向であったため,肝円索膿瘍との鑑別に苦慮した1例であった.
