We report the endovascular treatment of a dural carotid cavernous fistula in a 67-year-old woman in whom superior petrosal sinus catheterization was performed to access the venous site of the fistula. To our knowledge, this retrograde venous route via the superior petrosal sinus has not been previously described.
Iatrogenic cavernous carotid pseudoaneurysms are a special group among other intracranial aneurysms. They can occur during the dissection phase of the surgery if the tumor encases a vessel. Complications of their rupture as hemorrhage or stroke are life threatening. Early recognition and treatment is mandatory to avoid catastrophic sequelae. We present the successful diagnosis and endovascular ...
A 40-year-old woman presented with symptoms compatible with Raeder syndrome. MR demonstrated narrowing of the left cavernous carotid artery. The high cervical portion of the left internal carotid artery was not identified. Angiography confirmed the narrowing of the cavernous carotid artery and showed marked and irregular stenosis of the distal cervical internal carotid artery. Involvement of th...
The authors studied the microsurgical anatomy of the suboccipital region, concentrating on the third segment (V3) of the vertebral artery (VA), which extends from the transverse foramen of the axis to the dural penetration of the VA, paying particular attention to its loops, branches, supporting fibrous rings, adjacent nerves, and surrounding venous structures. Ten cadaver heads (20 sides) were...
Acute abducens nerve palsy as a presenting feature in carotid-cavernous fistula in a 6-year-old girl
Carotid-cavernous fistulas (CCF) are abnormal communications between the internal carotid artery and the cavernous sinus. Traumatic carotid-cavernous fistulae are rare potential complications of craniofacial trauma. Typical findings of CCF are proptosis, chemosis, headache, oculomotor or abducens nerve palsy, trigeminal pain and pulsating bruit over the temporal skull and the bulb. CCF are repo...
An 11-month-old female patient presented with symptoms of orbital venous congestion. A CT scan showed enlarged superior ophthalmic vein and cavernous sinus on the symptomatic side. Catheter angiography confirmed a direct carotid-cavernous fistula with intra- and extracranial venous drainage. Successful coil embolization of the fistula was performed with resolution of the infant's symptoms and r...
A 40-year-old woman was admitted to a local hospital following the acute onset of severe right temporal and retroorbital headache associated with nausea and vomiting. For 2 or 3 days before the onset of symptoms she had experienced a flulike illness with repeated episodes of coughing. She also had a mild temperature elevation. Her past medical history was unremarkable. At the time of initial ex...
Anatomically the cavernous sinus is closely related to the sphenoid sinus and the nasopharynx. Sphenoid sinusitis, as demonstrated in the case can affect the nerves which are contained within it. Apart from the sinus, a direct extension from a nasopharyngeal lesion is commonly encountered to cause diplopia, owing to the sixth nerve involvement. The provisional diagnosis should be a nasopharynge...
Intracranial aspergillosis is rare in immunocompetent patients. Extension to the cranial base via the sphenoid sinus is exceptional. Authors report a case of middle cranial fossa aspergillosis involving the cavernous sinus and mimicking an extra-axial tumor in an immunocompetent 65 year-old male. The patient had been suffering from left trigeminal neuralgia and a horizontal diplopia since 4 mon...
