We report a case of visceral leishmaniasis (VL) in an immunocompetent native from non-endemic region of India that presented with chronic diarrhoea. VL was not a differential diagnosis and was unexpectedly diagnosed as intestinal leishmaniasis through the identification of the Leishman-Donovan (LD) bodies in duodenal and colonic mucosa. The patient expired before receiving antileishmanial therapy.

Post kala-azar dermal leishmaniasis (PKDL) is a skin manifestation that usually develops after treatment of visceral leishmaniasis (VL), a major public health problem in India. The diagnosis and management of PKDL is complex. This is the first case report from India in which PKDL occurred after paromomycin treatment for VL in an Indian patient.

Exoantigens (exo) from Leptomonas seymouri and Crithidia fasciculata were used in an enzyme linked immunosorbent assay (ELISA), showing 100% reactivity with sera from visceral leishmaniasis (VL) cases, and no reactivity with American tegumentary leishmaniasis (ATL) ones. Our results have indicated that these exoantigens can be applied in the discrimination of VL and ATL cases.
