Most cervico-mediastinal goiters are situated in the anterior mediastinal compartment, but according to the literature, 10-15 percent of them are located in the posterior mediastinum. Although most anterior mediastinal goiters can be removed by using the transcervical approach, cervico-mediastinal goiters in the posterior mediastinal may require additional extracervical incisions. We report the...
We report an unusual case in which a patient presented with a large posterior mediastinal goiter that extended to the level of the aorta. The goiter was resected through a standard Kocher neck incision with mediastinoscopic assistance. The large goiter was completely excised without the need for a sternotomy.
Surgical removal of intrathoracic goiter can be performed by a cervical approach in the majority of patients. Review of literature shows that experienced surgeons need to perform an extracervical approach in 2-3% of cases. In spite of surgical management of substernal goiter is well defined, there is little available information about surgical approach of intrathoracic goiters extending beyond ...
Because of their tendency to progressively enlarge with compression of adjacent structures, as well as the small chance of malignancy, most intrathoracic goiters should be excised surgically. Most anterior substernal goiters and some ipsilateral posterior mediastinal goiters can be removed safely through a cervical incision. Large posterior mediastinal goiters, contralateral retrotracheal or re...

The radiologist needs to be aware of a potential pitfall that can influence the imaging appearance of thyroid goiter. Whether the patient is imaged with the arms overhead or by the side may affect the apparent mediastinal excursion of a goiter. CT scans obtained with the patient's arms by the side are more accurate for determining substernal extent of goiter than when the arms are overhead, a p...
