永久気管孔に近接した咽頭皮膚瘻に対してボイスプロステーシス(プロヴォックスⓇ:PV)を留置することで経口摂取が可能となった症例を経験したので報告する。 症例は73歳男性。喉頭声門癌T2N0M0に対して放射線単独治療67.5Gyを施行した。再発のため単純喉頭全摘術を施行したが気管孔近傍に瘻孔を認めた。初回手術から85日目に大胸筋皮弁で瘻孔の閉鎖を施行したが,気管孔側より再度瘻孔を認めた。その後は瘻孔にTチューブやレティナカニューレを用いながら,最後はPVを留置することで気管孔の妨げにならず十分な経口摂取が可能になった。 気管孔に近接する咽頭皮膚瘻に対してPVは選択肢のひとつになりうると考える。
